Job Details

This Community Care Position is a travel position that covers Jackson and Josephine Counties. On days that you will not be in community care, you will assist at a local practice.

JOB SUMMARY:
Provides dental hygiene services to patients outside of Advantage Dental from DentaQuest's practices within local communities through approved community health promotion programs (ex. WIC, Head Start, etc.);
Collaborates with the Manager of Community Care and Advantage Dental from DentaQuest's community partners to provide onsite dental health prevention and promotion of services to individuals, families and community members within a designated region/community and in accordance with any applicable health promotion program(s). Works with corporate office to assess, plan, implement and evaluate community outreach programs and services for children and their families. Also expected to work collaboratively with third-party professionals, agencies, community members, and groups to assist local communities in meeting their dental needs. May attend community outreach meetings as needed. Expected to perform outreach tasks independently and with minimal supervision, while maintaining company standards, brand and mission.
Provides dental hygiene services to patients within the Advantage Dental from DentaQuest's network on a consistent but limited monthly basis not to exceed 20 percent of their time. When performing work within the clinic environment, expected to assess patient needs, and plan and deliver direct dental care and associated services to patients while under the supervision of a qualified dentist.

PRIMARY JOB RESPONSIBILITIES:
Community (80-100%)
Management of Community Care Program within assigned region.
Independently perform outreach tasks.
Make decisions to optimize Community Care Program within assigned region.
Conduct dental health surveys, screenings and caries-risk assessments.
Educate individuals, families and groups about the importance of oral health.
Provide fluoride treatments.
Manages and represent Advantage on local and regional community health committees.
Research, develop, and evaluate dental resources for clients, professionals and communities.
Participate in multi-disciplinary community activities.
Review literature and conduct research related to dental health care.
Work effectively with children, families, aged populations, persons with developmental disabilities, community groups and inter-disciplinary teams using health promotion principles.
Assess and provide oral health services to patients with complex dental needs.
Collaborate with network dentist to provide expanded practice services according to OAR 818-035-0065 and 818-035-0100.
Collaborate with Manager of Community Care and Case Management team on community outreach projects.
Coordinate with community partners on scheduling outreach activities.
Work with case management and community partners to ensure follow through for dental treatment and return of consents are received.
Any other duties that may be assigned.
Practice (0-20%)
Assesses dental conditions and risk, uses patient screening procedures, to include medical history review, dental charting, and Perio charting; takes patient vital signs as required.
Follow all state dental practice acts to deliver direct care to patients as assigned and using established dental hygienist procedures; performs routine treatment procedures, such as cleaning, polishing, x-rays, application of sealants, fluorides and varnishes, and related procedures.
Perform periodontal maintenance and other periodontal procedures as indicated.
Develops and implements individualized dental care plans for patients; performs patient education and risk protocols, discharge planning, and patient/family teaching under the direction of a dentist.
Provides chair-side assistance to dentist in the performance of special tests, procedures, and complex treatments.
Makes impressions of patients' teeth as needed.
Documents dental history of chief complaint; records and reports pertinent observations and patient reactions to dental team, as appropriate; documents lab procedures and ensures follow up on result.
Assists with or institutes emergency measures for sudden adverse which may occur during treatment.
Performs patient triage and initiates patient care as appropriate for all patients.
Assists in preparation of patient care areas, and in the patient admission, transfer, and discharge process, as required; prepares reports and assists as required with patient reception, telephone calls, and other office duties.
Performs miscellaneous job-related duties as assigned.
Assumes responsibility for the upkeep of the dental operatory and equipment; promptly notifying practice manager of any problems which may affect their ability to deliver safe and timely dental care.
Performs routine cleaning and maintenance of dental equipment in assigned operatories and for the assigned equipment. Regularly and thoroughly performs maintenance of assigned equipment consistent with the upkeep of the other treatment rooms.
Ensures use of downtime to include cleaning and stocking operatories as well as helping the rest of clinical team as needed.
Attends and participates in team meetings and required training programs.
Manages the schedule in collaboration with the Practice Manager.

JOB QUALIFICATIONS:
Associate's or Bachelor's degree in Dental Hygiene.
Oregon RDH license.
Local anesthesia permit.
Expanded practice permit.
Ability to work independently
Management and Problem Solving Skills
Demonstrated experience placing sealants preferred.
Current CPR certification.
Full-Time schedule required.

TRAVEL REQUIREMENTS:
Ability to travel using personal car as required by the company.
Occasional overnight travel.

PHYSICAL DEMANDS:
Continually (75% or more): Sitting; keyboard operation; use of clear and audible speaking voice; ability to hear normal speech level in person and over telephone.
Frequently (50%): Standing and walking; grasping/squeezing; lifting, carrying, pushing or pulling 1-25 pounds.
Occasionally (25%): Bending, squatting, kneeling, reaching overhead; lifting, carrying, pushing or pulling 25-50 pounds.
Rarely (10%): Climbing stairs, climbing ladder/step-stool (varies by area); crawling; operation of a motor vehicle.
The ability to bend, squat, kneel, reach overhead and carry or lift up to 50 pounds.
The ability to handle sharp instruments.
May be exposed to bloodborne pathogens, chemical hazards, and respiratory hazards in a controlled environment.
May be exposed to hazardous chemicals or substances.
Personal Protective Equipment required.
